CONCORD — The New Hampshire Department of Health and Human Services recently released a detailed plan for new models of delivery and payment of nursing facility and home and community-based long-term supports and services.

The plan will offer a choice between managed care and locally based and operated programs for all-inclusive care for the elderly or PACE Centers.
