CONCORD — Gov. Kelly Ayotte and the New Hampshire Department of Health and Human Services are seeking public input from rural patients and residents, community leaders, hospitals, health centers, community mental health centers and partners across New Hampshire to shape a bold, new plan to strengthen care and services for every rural community in our state.

Federal legislation passed earlier this year established a $50 billion Rural Health Transformation Program administered by the Centers for Medicare and Medicaid Services.
