BEDFORD — N.H. Healthy Families, a leading N.H. Health Plan serving Medicaid and Marketplace members will partner with Coos County Family Health Services to bring its Green to Go! van offering fresh fruits and vegetables, lean meat, nutrition education, resources and support to the Berlin community at no cost.
N.H. Healthy Families offers nine distinct programs to promote proper nutrition and food security for its members and the broader community. In addition, its clinical team leads several initiatives focused on food security. These include health coaching programs that provide education and support for nutritional needs, post-discharge meal services that deliver medically tailored meals to members—particularly those with chronic conditions—after a hospital stay, self-monitoring programs, and enhanced transportation services that connect members to non-profit resources, such as local food pantries.
