(BPT) - Long-term health conditions are playing an increasingly significant role in the lives of Americans. Chronic diseases, such as diabetes, cardiovascular disease, cancer and hypertension, are impacting the health of millions of Americans and becoming increasingly common.

In the U.S. alone, three in four adults have at least one chronic disease, according to the Centers for Disease Control and Prevention (CDC). As these numbers continue to rise, it's more important than ever that people take steps to reduce their risk of developing chronic diseases.
